How to register with Bridge Surgery

To register with the surgery:

  • fill out an online registration form by clicking here
  • download a paper registration form by clicking here and take it to reception from 8am to 6pm
  • visit reception from 8am to 6pm and collect a paper registration form

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

New Patient Registration

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

Once Registered

Once you are registered you will be allocated a named GP Partner. You will be advised of your assigned GP once the registration process is completed. You can express a preference about which GP you are assigned to should you wish, and also which GP or clinical professional you see when booking an appointment (let our Reception Team know), however you may not always we able to see the same GP or clinical professional if they have no availability on the day you need your appointment.
